The MHRA has issued a drug alert on the products listed below. Teva UK Ltd is recalling the below batch due to testing of this product has shown that some samples were marginally out of specification.

Aurum Pharmaceuticals Limited (trading as Martindale Pharma) has informed us that there is an error on the syringe label of the above batches associated with the scale of gradations which show nominal volume at 0.1ml intervals.

Special Products Ltd carried out several modifications to this unlicensed product in December 2011. Information about the changes appears not to have been adequately passed on to users of the product and has the potential to result in product overdose.

Class 4 Drug Alert (Caution in use): Clexane S PFS Inj 40mg X10(S/Lock)
Sanofi-aventis have informed us that the needle caps on some syringes of the below batch require more force than usual for removal. The affected syringes can still be used but users are advised to be vigilant when preparing syringes from this batch for use. The quality of the contents of the syringes is not compromised by this finding.

Class 4 Drug Alert (Caution in use): NuTRIflex Lipid
Further to the previous drug alerts we issued on 21 December 2011 and on 11 January 2012, B.Braun Medical Ltd have advised us that they need to release additional batches of the below NuTRIflex Lipid products which may contain particulates and should be subject to the same additional filtration process previously recommended during infusion.

This is a supplier led recall. Reckitt Benckiser (UK) Ltd is recalling the affected batches below due to a small portion of this product has been exposed to cold temperatures, which has resulted in non reversible solidification of the contents.

Accord Healthcare Limited is recalling the above batch because the blister foil in some packs is incorrectly labelled as Amoxicillin 250mg. All other labelling is correct and the capsules present in the blister are 500mg strength.
